Staff COVID-19 Reporting Form

Please note this is the Staff form.  If you are a Student, please use the link to the Student Form.

A copy of this form will be sent to your line manager, the Director of Operations/Head of Division for your area, and the Staff Health & Wellbeing Team.  However, you should also verbally notify your line manager.  In addition to completing this form all sickness absence (including where this is due to COVID-19) must be recorded on SAP (ESS or MSS) as either:

*     Coronavirus – confirmed by health professional or
*     Coronavirus – suspected in line with the latest guidance on symptoms.

The University needs to collect information for all staff, including Unitemps workers, who:

  • Have received a positive test result for COVID-19 or
  • Are isolating as a consequence of the latest government guidance.

Please note this form does not replace your responsibilities in respect of the NHS Test and Trace process.
