I am sorry but I have been ill with ………….. and have not been in lectures (labs/seminars) since  DATE.  I have not been able to keep on top of all my work and the following piece(s) are likely to be late.  I have asked the Medical Centre for a doctor’s letter.   Where should I take the letter when I get it?

I know there are forms that I will need to fill in when I return.  As I have not done one before it is likely that I will need support to complete it accurately.   Who should I see about this? Where will I find them and at what times are they available?
