Example form

This form demonstrates all the different types of items (form fields) that can be added to a form. Feel free to fill in the form and submit it to try it out.

This area is call Form Prologue to access it on your form:

You can apply styles to text in this area to improve its appearance

Throughout this form 'responder' refers to the person who completes the form, questionnaire or application.

 

To create the form field items below on your form click Add New.  To edit the form field items on your form, click the spanner in the top right corner to change to edit mode.

(Required)
This field is automatically created with the form. It fills in the respondent's email address automatically. You can rename it and move it
This gives a single tick box. The 'display string' can be changed eg to read 'Yes' when true and 'No' when false. This information will only show when the form is emailed back to you. This field can be left as default or set as Required with a Validator eg "Tick box to confirm you have read the terms and conditions" The respondent will then be unable to submit the form without ticking the box
This allows the respondent to fill in the form using a calendar or by choosing the date. Drawback: The time must be chosen from each of the pull-downs separately

This field specifies that a number must be added and allows a maximum value to be set - for this field the maximum is 30. The size of this field has been set to 5 - this controls the width of the box.
Fieldset folder
Don't try until you have gained experience of using standard forms. This is a folder which groups other form fields as a fieldset. To add new form fields within it: edit the Form Fieldset Folder | change to View | Add New. Using a fieldset folder does add two extra levels to your structure and requires additional clicking and editing to amend the fields.

This allows files to be uploaded. A maximum size can be specified.
This is just a lable - text you can add to your form. There is no input component.
This appears as a text area which allows you to specify the number of rows visible.
This field is a selection list or a checkbox list which allows more than one response. Type the responses into the options - 1 per line. This is a selection list. You must instruct the respondent to use Ctrl + click to select more than one.

A 'Likert' form entry - questions with ranked responses. Drawback: you cannot control the width of the columns

Rich Label Field - this allows you to add a label using styles and formatting.  There is no input component.

This allows the recipient to fill in the text and format it. Drawback: the size of the box can't be controlled. This is the standard size.
This allows you to add a pull-down selection field or radio buttons - only one response is possible. This is a selection list.

This is a single line text field - the maximum number of characters can be specified. The width can be specified by number of characters. This field is set to 50. A validator can be added eg "is a well formed URL"
This allows the respondent to add free text. The height can be specified by number of rows - this is set to 3 rows. The maximum length can be specified.

Required fields

If required is ticked, the form cannot be submitted until some information has been entered. 

Validated fields

If validation has been added eg number must be lower than 10, the form cannot be submitted if invalid information has been entered.

The form will highlight the invalid field if the respondent attempts to submit.

Fields will be listed in the order they are added.  They can be reordered in edit mode.  If you have more than a page length of fields click onto the Contents view | if the fields go onto two pages click Show all items | move the fields by clicking and dragging the four dots in the order column.
